It includes 236 items and covers general health, vision, hearing, cardiovascular health, orthopedic health, neurological health, health habits, medical history, and more. Screens for medical referrals and serves as a health information resource. There is no formal scoring procedure. It is not a substitute for a medical evaluation; this instrument should provide a more detailed database from which the clinician or therapist can make an appropriate referral.
